Cher Wee leads MOHT’s collaborations in developing shared care approaches that strive towards delivering frictionless acute-to-community transitions. Such approaches seek to reduce fragmentation and improve outcomes for patients with multiple health and/or social issues, by enabling informed choices. In the transitional care space, for example, Cher Wee’s team has been developing a set of customisable digital capabilities for data assembly and sharing across settings and care teams. His team has also been supporting substitutive approaches like Hospital at Home, where targeted lower acuity conditions could be right sited safely in the community while still acutely unwell. Prior to joining MOHT, Cher Wee served at Singapore’s public hospitals in a few roles. He led frontline staff in clinical operations support, analytics, infrastructure redesign, and process improvements.
